Special Monthly Compensation: Aid & Attendance and Housebound Benefits

Overview

Veterans requiring daily living assistance or confined to their homes may qualify for additional Special Monthly Compensation (SMC) or Special Monthly Pension (SMP) beyond standard disability payments.

Chief's Take: Aid and Attendance and Housebound represent SMC tiers for veterans needing help with basic activities or who cannot regularly leave home. The eligibility standards are precise, and the additional monthly compensation is substantial. Determine whether you qualify.

Aid and Attendance Defined

Aid and Attendance benefits compensate veterans who require another person's help with routine daily activities: - Bathing and personal hygiene - Dressing and undressing - Feeding and meal preparation - Managing prosthetic or orthopedic devices - Avoiding hazards in their daily environment

Housebound Defined

Housebound status applies to veterans substantially unable to leave their residence due to disability severity.

Eligibility Criteria

SMC Aid and Attendance Requirements

  • Service-connected disability exists
  • Regular assistance from another person required
  • OR permanent bedridden status
  • OR severe blindness

SMP Aid and Attendance Requirements

  • VA pension eligibility criteria met
  • Regular assistance from another person required
  • OR permanent bedridden status

Application Steps

Step 1: Form Completion

VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance)

  • Sections I through V: Complete yourself
  • Sections VI through VIII: Your primary care physician completes

VA Form 21-0779 (Request for Nursing Home Information) - Required for nursing home residents - Sections I through III: Complete yourself - Sections IV and V: Nursing facility administrator completes

Important Application Notes: - The claimant is whoever needs Aid and Attendance or qualifies as Housebound - Block 13 requires selecting the specific benefit type requested - If your physician declines form completion, submit VA Form 21-526EZ to request a VA examination

Step 2: Documentation Submission

  • Upload forms through VA quick submit at eauth.va.gov
  • Include additional medical evidence strengthening your case
  • Submit VA Form 21-4142/a when you cannot obtain private records yourself

Processing Considerations

Unlike TDIU, Aid and Attendance applications are not automatically treated as increase claims. Thorough documentation typically eliminates the need for Compensation and Pension examinations, enabling faster decisions.

SMC Level Structure

Under 38 CFR 3.350, relevant levels include:

Level Qualifying Criteria 2026 Monthly Rate (approx.)
SMC-S Housebound: 100% schedular plus 60%+ independent additional disability OR substantially confined to dwelling $4,408
SMC-L Requires regular aid and attendance from another person $5,046
SMC-L1/2 A&A plus loss or loss of use of one additional extremity $5,302
SMC-R1 A&A plus need for in-home nursing-level care $10,420
SMC-R2 Requires healthcare services within a protected residential environment $11,956

Level Distinctions: - Housebound (SMC-S) differs from Aid and Attendance—it addresses home confinement, not daily care needs - SMC-L through SMC-O represent progressive levels based on disability combinations - SMC-R levels provide the highest compensation for intensive care requirements
